北京大学医学部肾脏疾病大数据研究中心

北京大学医学部肾脏疾病大数据研究中心成立于2017年12月14日,挂靠北京大学健康医疗大数据研究中心。早在2014年,“中国肾脏病学之母”王海燕教授就提出了建设“中国肾脏疾病数据网络”(CK-NET)的构想,并引领北京大学肾脏疾病团队做出了大量工作、获得国内外的广泛认可。近3年来,随着“大数据”理念和技术的出现,CK-NET得以高速发展。当前在发展“北大医学”的大背景下,本中心由北京大学肾脏病学系和北京大学健康医疗大数据研究中心共同建设。
研究领域及方向为:
肾脏疾病监测及负担研究
1)监测我国肾脏疾病的发病情况和疾病特征;
2)对肾脏疾病的疾病负担深入探究;
3)构建肾脏疾病标准术语和知识图谱;
肾脏疾病防控策略研究
1)提出肾脏疾病防控策略,为相关政策制定提供建议,开展卫生经济学评价和政策仿真研究;
2)建立肾脏病临床辅助决策支持系统和风险预测模型;
肾脏疾病临床科研问题
聚焦临床实践中提出的重要科学问题,提升传统研究的效率和质量,通过与其他领域数据(如环境)交叉、更深理解肾脏疾病的发生与进展。
中心由北京大学第一医院肾内科赵明辉主任总体负责,北京大学肾脏疾病学系与北京大学健康医疗大数据研究中心的团队成员提供全力支撑。各研究方向均有学术带头人等高层次水平人员为主导,同时注重年轻专业人员参与,形成结构合理、有持续发展性的人员梯队。中心成员包括国家自然科学基金杰出青年2人,国家自然科学基金优秀青年1人等。
中心已具备完备的仪器设备和满足正常科研需求的场地用房。目前已建立用于存放服务器和数据资源的机房,可满足大数据分析的硬件条件;同时已安装监控摄像头、具有完整规范的数据管理、申请和使用流程。此外,北京大学健康医疗大数据研究中心也正在建设以符合国家等级保护三级为目标的机房,具备足够的数据储存和分析能力。
目前可用的数据涵盖了超过100万人群的若干大型数据库,包括国家级管理型数据、社保数据、多中心队列研究数据和区域性电子病历数据等。中心团队的研究工作“慢性肾脏病的疾病负担评价、危险因素探讨及防治策略研究”项目获2015年北京市科学技术奖三等奖、2015年中华预防医学会科学技术二等奖。
基于十余年积淀的人群研究数据、利用国家卫生监管性大数据平台,产出了发表在 N Engl J Med 的论文,揭示糖尿病肾病已经取代慢性肾小球肾炎,成为慢性肾脏病的首位病因,并于2016年形成政策建议提交国家卫生和计划生育委员会和世界卫生组织,建议我国肾脏疾病的学科建设应该进行调整、增加对于糖尿病肾病的重视。另一主要成果是中国肾脏疾病年度报告(Annual Data Report,ADR)。2014年ADR主要囊括了中国透析前住院患者,包括描述慢性肾脏病患者不同疾病特征的8个章节,2个急性肾损伤章节,以及1个肾移植等待名单章节。整个ADR于2017年6月在 Am J Kidney Dis 的增刊上全文发表(http://www.ajkd.org/issue/S0272-6386 (16) X0003-0),并且获得了中华医学会肾脏病学分会现任主任委员余学清教授的高度认可。

Center for Data Science in Kidney Disease, Peking University Health Science Center
The Center for Data Science in Kidney Disease, Peking University Health Science Center was founded in December 14, 2017, affiliated to the Center for Data Science in Health and Medicine, Peking University. The concept of China Kidney Disease Network (CK-NET) was initiated by the late Professor Hai-Yan Wang in 2014, and is further developed with the emergence of “Big Data” during the last 3 years. Currently the Center for Data Science in Kidney Disease is coordinated by the Peking University, at both the Department of Nephrology and the Center for Data Science in Health and Medicine under the background of the development of "Peking University Medicine".
Our current research directions include:
1. Monitoring and burden of kidney disease
1) monitoring the incidence and characteristics of kidney disease in China;
2) exploring the disease burden of kidney disease;
3) constructing the standard terminology and knowledge atlas of kidney disease;
2. Prevention and control of kidney disease
1) putting forward the prevention and control strategy of kidney disease, providing suggestions for relevant policy formulation, and carrying out health economics evaluation and policy simulation research;
2) establishing the clinical decision support system and risk prediction model for kidney disease;
3. Clinical research of kidney disease
Focusing on the important scientific problems in clinical practice, improving the efficiency and quality of traditional research, and further understanding the occurrence and progress of kidney disease through data integration with other fields (such as environmental science).
Professor Minghui Zhao from Renal Division, Peking University First Hospital is the director of this center. The team members of the Department of Nephrology and the Center for Data Science in Health and Medicine, Peking University will provide full support for the development of the center. Academic leaders are dominant in all research directions, with the participation of young academics to form a rational and sustainable staff echelon. The members include 2 persons honored with the National Science Fund for Distinguished Young Scholars and 1 person with the National Science Fund for Excellent Young Scholars.
The center has complete equipment and room to meet the needs of scientific research. At present, a computer room has been set up for storing servers and data resources, which can satisfy the hardware condition of big data analysis. A surveillance camera has been installed, with the standardized data management, application and utilization process. In addition, the Center for Data Science in Health and Medicine is also building a computer room that is in line with the National Three Level Protection Standard, with enough data storage and analysis capabilities.
Currently, several large datasets involving over 1 million individuals, including national administrative and claims datasets, multi-center cohort studies, and regional electronic health records, are available for use. The previous research project, “Study on disease burden, risk factors and prevention strategy of chronic kidney disease” won the third prize of 2015 Beijing Award for Science and Technology, and the second prize of 2015 Award for Science and Technology of Chinese Preventive Medicine Association.
Based on the accumulated population data for more than ten years and the national health supervision data platform, our paper published in N Engl J Med revealed that diabetic kidney disease has replaced chronic glomerulonephritis, becoming the first cause of chronic kidney disease. The corresponding policy recommendations were delivered to the National Health and Family Planning Commission and the World Health Organization, suggesting that the discipline construction of kidney disease in China should be adjusted to increase attention of diabetic kidney disease. Another major output is to generate the Annual Data Report (ADR) regarding kidney disease in China. The 2014 ADR included mostly pre-dialysis hospitalized patients in China. Eight chapters describing various aspects of patients with chronic kidney disease, as well two chapters involving acute kidney injury and one chapter regarding kidney transplantation waiting lists, are included in the ADR. The whole ADR has been published as a supplement of Am J Kidney Dis in June, 2017 (http://www.ajkd.org/issue/S0272-6386 (16) X0003-0); and has been endorsed by the current president of Chinese Society of Nephrology, Professor Xueqing Yu.
