痴呆诊治转化医学研究北京市重点实验室

   

  实验室痴呆诊治基础与临床研究工作最早始于上世纪八十年代由我院组织的全国首次痴呆流行病学调查。并在1990年由学术奠基人沈渔邨院士牵头组建老年精神疾病研究组,1998年医院开设记忆障碍专业门诊服务,2001年正式成立记忆障碍门诊服务团队,2007年老年学科组的提议下,正式挂牌成立记忆障碍诊疗与研究中心,挂靠于临床研究室,2009年本团队联合全国17家医院组建全国痴呆诊治与研究协作网络,并在2014年成功申请痴呆诊治转化北京市重点实验室。

  痴呆诊治转化医学研究重点实验室定位于“以临床推动研究,以研究引导服务”,以痴呆诊治生物标记物的基础研究与应用转化为主要方向,开展痴呆诊治策略的应用基础研究。主要研究内容包括:检测痴呆不同病理机制路径的生物学标记物,探索识别不同类型痴呆的神经影像与分子生物标记物;探索痴呆的发病机理,在细胞与动物模型中验证生物标记物;验证诊断标记物在多中心临床实践中的诊断价值与可行性;评估生物学标记物在公共卫生服务体系的转化价值,筛选适用于医院诊治与社区筛查的早期诊治生物学标记物检测技术与评估体系。通过这四个方面的研究,实验室将筛选出适宜的痴呆早期诊治指标体系,提升北京市及全国的痴呆早期诊治水平。

  痴呆诊治转化医学研究北京市重点实验室依托北京大学精神卫生研究所(第六医院)、卫生部精神卫生学重点实验室以及国家精神心理疾病临床医学研究中心,已构建完善的痴呆临床与基础研究体系和全国规模最大、包含多模态神经影像学资料、外周血及全面神经认知与精神行为评估的标准化病例登记库,现已成为全国最大的痴呆研究及临床试验基地之一。实验室拥有工程院与科学院院士各1名,并建立了以长江学者、杰出青年学者、博士生导师为学术带头人、青年科研人员为骨干的临床与基础研究梯队。自上世纪八十年代起,团队在沈渔邨院士的带领下,历时近30年,在国内率先开展了大量的痴呆诊治基础与临床研究工作,引进MMSE、ADAS-Cog等重要认知功能评估工具,广泛用于痴呆的临床研究、药物试验、非药物干预、照护以及公证鉴定等领域,建立了全国痴呆早期诊治的多中心协同研究网络和北京痴呆社区筛查技术推广工作。近年来,系统展开了痴呆的发病机制研究、神经生物标记物研究、临床转化研究和人群转化与社会服务研究。近年来主编及参编中英文著作8项,发表高水平论文72篇,获专利2项,教育部科学技术进步奖二等奖1项。痴呆诊治转化研究已达到全国领先水平,并具有国际一流研究水平接轨的实力。

  近年来的重要科研成果及贡献:

  发现APOE4等位基因对AD患者后默认网络和执行控制网络的脑功能集结中心和特定功能系统连接具有选择性调节作用;

  发现行为变异型额颞叶痴呆患者存在“皮层—纹状体—丘脑—皮层”神经环路的结构异常;

  发现血浆APL1β28及Clusterin水平可反映MCI的认知表现;

  发现AD患者gamma节律功率及高频节律与低频节律的相位耦合强度显著增加;

  开发并验证了老年期抑郁症症状清单(GDI);

  引进了国际痴呆照护分级评估量表(IDEAL);

  研发并推广了老年期痴呆医院-社区-家庭综合干预技术;

  发现痴呆患者首次到医疗机构就诊的时间滞后于其首次出现症状时间;为420名基层服务人员开展了老年期痴呆早期筛查和居家照护辅导技术培训,向逾万名社区老人提供了认知功能筛查和痴呆相关知识宣讲服务。

    Beijing Municipal Key Laboratory for Translational Research on Diagnosis and Treatment of Dementia

 

  In 1980's, the first epidemiological survey of dementia in China was conducted by Peking University Sixth Hospital, which was also the beginning of the basical research and clinical service in dementia diagnosis and therapy of Beijing Municipal Key Laboratory for Translational Research on Diagnosis and Treatment of Dementia .In 1990, the geriatric psychiatry research team was developed by the Chinese Academy of Sciences and Chinese Academy of Engineering academicians: Prof. Yucun Shen. In 1998, memory clinic started providing consultation, clinical service for patients with dementia. Hospital announced the official commencement memory clinic in 2001. In 2007, the hospital expanded the memory clinic into Dementia Care & Research Center. In 2009, We launched the Dementia Care & Research network which contain 17 hospitals in China. In 2014, Beijing Municipal Key Laboratory for Translational Research on Diagnosis and Treatment of Dementia was approved by the Beijing Municipal Science & Technology Commission.

        To stick to the resolve of “promoting researches via clinic and guiding service by researches“, Beijing key laboratory for diagnosis and intervention translational medical research of dementia diseases carries on a series of application and basic researches focusing on the biomarkers for diagnosis and intervention in patients with dementia. Our main interests include four domains as follows: 1)exploring and distinguishing neuroimaging and molecular biomarkers of dementia in different types, 2) exploring the underlying mechanisms of dementia and verifying the biomarkers in cell and animal models, 3) verifying the value and feasibility of biomarkers in multicenter clinical practices, 4)evaluating the translational value of biomarkers in public mental health services system, and screening the feasible biomarker-detecting techniques and evaluation systems for diagnosis and intervention in hospitals and early screening in communities. Focusing on the above researches, our lab will select the suitable biomarker systems for early diagnosis and intervention of dementia, to promote the level of diagnosis and intervention of dementia in Beijing and even the whole country.

  Beijing Key Laboratory for Translational Research on Diagnosis and Treatment of Dementia has established a complete set of clinical and basic research on dementia system and the national largest standardized database of multi-modal neuroimaging data, peripheral blood and comprehensive neuropsychiatric assessment, and have currently become one of the national largest research and clinical trial bases for dementia. The laboratory has an academicianof the Chinese Academy of Sciences (CAS) and an academician of the Chinese Academy of Engineering (CAE), and has also established a clinical and basic research team with Changjiang scholars, outstanding young scholars and doctoral supervisors as academic leaders and research backbones. Since the 1980s, under the leadership of academician Yucun Shen, the team has took the lead in conducting a large number of basic and clinical researches on the diagnosis and treatment of dementia in China for nearly 30 years. Primary cognitive function assessments tools have been imported early by the team, such as MMSE and ADAS-Cog Tools, and widely used in dementia clinical research, drug testing, non-drug interventions, care and notarization etc. In particular, the team has established a national multi-center collaborative research network for dementia early diagnosis and treatment, and also has developed and pushed the dementia screening technology to communities in Beijing. In recent years, the laboratoryhas systematically started the study of the pathogenesis of dementia, neurological biomarkers, clinical research and population transformation and social services. In recent years, the team has edited and coedited 8 Chinese and English books, published 72important papers, obtained2 patents and 1 second prize of Science and Technology Progress Award of the Ministry of Education. The laboratory has currently reached the leading level of the country, and been with the world-class level in dementia research

  Important scientific achievements

  Found APOE E4 selectively weakened the functional connectivity of rich-club regions and connections in the posterior default mode network and the executive control network in patients with AD;

  Found the structure of cortico-striatal-thalamic loop circuits was compromised in behavioral variant of frontotemporal dementia;

  Found APL1β28and clusterin plasma levels may reflect cognitive function in aMCI patients;

  Found gamma rhythm power and the cross-frequency coupling strength between the beta/gammaand low-frequency bands enhanced in AD patients;

  Developed and verified Geriatric Depression Inventory;

  Introduced the International Dementia AllianceSchedule;

  Developed and generalized Hospital-Community-Family-based comprehensive intervention for dementia;

  Found the median duration from the first noticeable symptom lagged behindthe first visit seeking diagnosis or treatment;

  Trained 420 general practitioners and social workers about dementia recognition and care giving and provided cognition screening and education for more than ten thousand older adults.
